Get Exe Name of Specified Thread / Window

I want to get the application exename + path, every window when get focussed
?

Does window have security on this kind of information ?
Using winsight, some window, do not have any information. at all (i.e.
follow focus)
If it is a security from windows, are there any way around to get such
information
