Where does NOT mean NOT?

When searching with Google, including "NOT somekeyword" or
"-somekeyword" (without the quotation marks, of course) in the search
criteria does not categorically exclude those pages that include the
word "somekeyword".

Do any search engines apply the submitted search criteria with greater
discipline than Google?
